About Making it Real
What’s Making it Real all about?
Making it Real has been co-produced with: people who have experience of accessing health, social care and housing services; Think Local Act Personal (TLAP) and the Coalition for Collaborative Care (C4CC). People with lived experience have been involved throughout, together with a large number of organisations.
It's built around six themes to reflect the most important elements of personalised care and support. Each theme has a number of 'I' statements which describe what good care looks like from an individual perspective. These are followed by 'We' statements which express what organisations should be doing to make sure people’s actual experience of care and support lives up to the 'I' statements.
The six areas of Making it Real:
- Living the life I want, keeping safe and well
- Having the information I need when I need it
- Keeping family, friends, and connections
- My support, my own way
- When things need to change
- The people who support me
Making it Real can help:
- The ‘I’ and ‘We’ Statement describe what good looks like from an individual’s perspective and what organisations should be doing to live up to those expectations
- The statements can be used to inspire and motivate organisations to keep getting better at what they do
- Help people talk to those who work in services so everyone can help make things better
- People and groups to think about what’s happening in their local area
In 2013 Making it Real Shropshire was established as part of a national initiative to help individuals to live more independently, and to choose and control how their support package is delivered.
