ASC PFA Team

The Shropshire Adult Social Care (ASC) Preparing for Adulthood Team (PFA Team) was established in 2018. It's a virtual team supporting young people across Shropshire who have adult care and support needs. It works with a range of partners within the council, across health services and with third sector organisations to deliver person-centred support to young people and their families which focuses on developing independence and living enjoyable, fulfilling lives.
