The Adult Social Care Preparing for Adulthood Team work with young people to develop a personal centred care and support plan.
A support plan is a document that details the support that your young person will need to ensure that they're living a happy and healthy life. The young person's worker from the PFA team will complete this with the young person and their family. The support plan will include all the support they'll need, including support from family, friends, available services in the community, or specific services that are funded by social care.
The support each young person receives is specific to them, is developed to promote independence and to support young people explore, and work towards their own aspirations for the future.
