Educational setting

In order to make sure that a request for an EHC needs assessment contains sufficient information for the panel to make an informed decision, all requests should include the following as applicable:

  1. School request form
  2. Person-centred plan - ADPR (assess, plan, do, review)
  3. Costed provision map (this is available in Excel format) or equivalent showing all the provision that the school makes which is additional to and different from that which is offered through the school’s curriculum the SEN Support Team
  4. Social care questionnaire
  5. Medical questionnaire
  6. One-page profile
  7. All About Me/All About Us (please read the example/hints document if needed)
  8. Most recent Educational Psychologist record of involvement and/or report – if available
  9. If appropriate details of any health involvement - including any school nurse involvement
  10. Any other professional advice sought which has informed recommendations/identification/provision
  11. If appropriate a whole family assessment, and early help plan. Find out more about the early help process or where a child is known to social care and has an allocated social worker please provide details of the social worker.

How to send your request to us

We ask that you send your request for an EHC assessment via encrypted email to senrequestsforassessments@shropshire.gov.uk. Please ensure that you first read the guidance on how to do so. To aid this process, forms are all available as Word documents, which can be completed electronically. An electronic confirmation of signatures will be sufficient, providing that the original signatures are retained on the school premises. Any additional paperwork will need to be scanned. 

What happens next?

The request and attachments will be sent through to the SEN team securely. A panel will decide, based on the information provided within the request, whether a statutory assessment should be undertaken. If the statutory assessment does go ahead, the child/young person and family will be assigned an assessment coordinator who will coordinate the assessment process and provide advice to support the family during this assessment period. If the outcome of an assessment is an EHCP, this plan will be completed and issued within 20 weeks of the date the SEN Team received the request for assessment.
