Phased Transfers
For parent carers of those children who are at a key transfer phase of their education, i.e. Y6 to 7 and Y11 to 12 Plus any unresolved transport queries.
Changes of settings or phase transfer:
This is both an exciting time and also a time when anxieties about next steps for your child’s education can rise.
The EHCP Team have working really hard to make sure that children, young people and their families are aware of their next educational setting in readiness for September.
The EHCP Team will be working all summer, and our focus will be on securing destinations for you. There will be some children, young people and their families who don’t yet know, for sure, where they are going in the new academic year. This will be for a number of reasons.
- Some children and young people are undergoing a needs assessment (EHCNA) right now, and there could be delays with reports being received by the team. Until all of the relevant reports are received, we cannot draft a plan and so cannot therefore consult with settings.
- Some needs assessments are completed and are waiting for the draft to be written. We can assure you that we are making those a priority and these will be issued to you just as soon as we can.
- Those Year 11 students for whom we have secured destinations, may have a ‘lag’ in their updated EHC Plan being issued to them. However, our focus has been on making sure that we have secured a college place or next steps placement for those students and the paperwork will catch up. A small number of students have yet to have their Year 12 placement confirmed. Your Case Officer will be keeping you updated of the work they are doing. - Some Annual Reviews* have been completed with recommendations for a change of placement. We acknowledge that these have not been dealt with in the timeliest way in some cases, but we are diligently working through those cases.
*If your young person, or if you are working with a young person, who has an outstanding Annual Review from before 31/12/2024 and is currently in NCY11 and you have not received any correspondence from a member of the team, please get in touch;

Transport Queries
Late placement agreements can also pose concerns about making suitable transport arrangements. Whilst we try our best to make necessary arrangements, we sometimes cannot get this in place to coincide with the Special Transport Team’s deadlines. If you are in this position, your application will still be assessed but the team may not be able to put arrangements in place for the start of term.
Once an application is approved and eligibility is confirmed parent/carers you will be contacted with the following:
· Travel options e.g. PTB, Council contracted vehicle
· Asked to complete a ‘Passenger Profile’
Please note the Special Transport Department cannot continue to process an application until they have confirmation from parent/carers of the type of assistance they would prefer, and the passenger profile is completed.
Once this information is returned it’s then a maximum of 20 days to procure and commission and inform families of travel assistance details.
