Community Navigator Service
PACC's Community Navigator Service offers personalised signposting and information provision for families of 0-25 years olds’ with SEND in Shropshire. This service is open to parent carers in Shropshire; a diagnosis is not required to access the Navigator Service. Our Navigators cover early years, school-age and preparation for adulthood (14-25). All of PACC’s Navigators are parent carers themselves, so also have lived experience of the SEND system in Shropshire across education, health and social care.
What does the Navigator Service cover?
PACC’s Navigators provide information and guidance across education, health and social care, including:
- Education options
- Managing finances and benefits
- Employment and volunteering opportunities
- Mental capacity and a parent carer’s role in decision making
- Social care
- The importance of friendships and peer relationships; being part of the community, accessing activities,
- PA support, Direct Payments
- Staying healthy - annual health checks, screening, activity levels and healthy eating
- Support planning and thinking ahead
- Accessing Services
- Support and self-care for parent carers
- Who can help when things become difficult?
How does the service work?
You will need to complete a short form to request a Navigator call, you will then be contacted via email to book an initial 30-minute teams or phone call. Our Navigators can be flexible with the time of appointments and can accommodate weekday evening calls if needed. Following your call, you will receive a follow up email with links to information and services that have been discussed.
