New directory provider application

Please note - this form is for new providers who aren't already listed on the Shropshire Together Community & Family Directory.

If you are already listed and want to update your service's details, please use this form to sign up for an OPD Provider account, or email directory@shropshire.gov.uk.

Our directory team will review all new provider applications and we will let you know if we can or cannot accept your request.

If accepted, the information you submit in this form will be used to set up your public provider listing on the directory. Once this is created, you will be contacted with further instructions on how to create and update listings for the services your organisation offers.

All details provided will be stored and processed in line with our privacy policy.
