Social prescribing self-referral form for adults

The healthy lives advisor will work together with you to develop a personalised action plan to improve your health and wellbeing. Follow-up support will be provided according to need and progress monitored.

Eligibility

To be eligible you must:

  • Be 18 years old or over
  • Live in, or be registered at a GP practice in South East Shropshire, South West Shropshire or Shrewsbury Primary Care Networks (The Beeches, Belvidere, Claremont Bank, Marden, Marysville, Mytton Oak, Prescott, Radbrook Green, Riverside, Severnfields, South Hermitage, Albrighton, Alveley, Bridgnorth, Broseley, Brown Clee, Cleobury Mortimer, Highley, Ironbridge, Much Wenlock & Cressage, Station Drive, Bishops Castle, Church Stretton, Craven Arms, Portcullis, The Meadows Clun)
  • Require additional support provided by social prescribing, in order to help and motivate you to take action to improve you health and wellbeing
  • Be able to communicate and engage with a Healthy Lives advisor and the activities on offer.
  • I confirm I am completing this form for myself as a self-referral (for referring organisations please contact healthylives@shropshire.gov.uk)

Registered GP practices not covered by the Healthy Lives Team are Shropshire Rural Alliance PCN practices and North Shropshire PCN practices (these practices have their own offer; please contact them directly (Pontesbury & Worthen, Clive, Knockin, Westbury and Shawbury, Cambrian, Caxton, Churchmere, Drayton, Hodnet, Plas Ffynnon, Wem & Prees).
