Parent/carer
What do I need to include with my request?
If you're a parent/carer requesting an assessment for your child/young person, please attach the consent form with your request. You may wish to also submit the one-page profile and/or any professional educational reports with your request, as they may aid us in any subsequent assessment process.
- Request for an assessment
- Consent form
- All About Me/All About Us document (this isn't a requirement at this stage but if you wish to complete it please read the example/hints document to help you fill in this form)
- Medical questionnaire
- Social care questionnaire
Where do I send my application?
Once you have completed your application please email the paperwork to senrequestsforassessments@shropshire.gov.uk
If you don't have the facility to print the forms and scan the paperwork to us, you can request paper copies of the documents to be sent to your home address and then you can complete these and post to:
SEN Team
Assessment request
Shropshire Council
Shirehall
Abbey Foregate
Shrewsbury
SY2 6ND
If you need to discuss your submission or have any queries, please contact the SEN Team on 01743 254267.
Alternatively, support is available from the Information, Advice and Support service, which offers advice and guidance on initial concerns about a possible SEN, through to more complex issues. Conferences, support groups, training, leaflets and other services are available, as well as one-to-one support.
What happens next?
The requests and attachments will be sent through to the SEN team securely. A panel will decide, based on the information provided in the request, whether a statutory assessment should be undertaken. If the statutory assessment does go ahead, the child/young person and family will be assigned an assessment coordinator who will coordinate the assessment process, and provide advice to support the family during this assessment period. If the outcome of an assessment is an EHCP, this plan will be completed and issued within 20 weeks of the date the SEN Team received the request for assessment.
