What will I need before I apply?

Completed application form including proposed DPS’ signed consent form
Proof of payment of the application fee
If you email your application to us we will forward it to the Responsible Authorities. If you post it you must also post a copy to each Responsible Authority (addresses on application form) on the same day you send it to us.

Download the form, complete it and email it to licensing@shropshire.gov.uk or print it out, fill it in and post your application along with the fee if applicable to:
