The Shropshire Wellbeing and Navigation Network
The Shropshire Wellbeing and Navigation Network is a preventative support service for adults aged 18 and over living in Shropshire. It helps people:
- Stay well
- Remain independent
- Feel connected to their community.
Support is available before problems become more serious and aims to reduce the need for crisis or statutory services. The service supports adults aged 18+ who may be:
- Finding it harder to manage at home
- Experiencing low-level mental health challenges
- Socially isolated or lacking confidence
- Recovering after illness or hospital discharge
- Living with long-term conditions
- Unpaid carers who need additional support.
The service is preventative and time limited. It's not a replacement for statutory care or clinical health services.
What support is available?
Support may include one or more of the following:
Supported hospital discharge
Delivered by British Red Cross, providing short term practical and emotional support for individuals and their family carers to help people return home safely and regain independence. This will include:
- Hospital discharge transport and accompaniment
- Immediate home resettlement support
- Short term practical daily living support (up to 6–8 weeks)
- Reablement and independence building support
- Home safety and assistive technology support
- Digital inclusion support
- Emotional wellbeing support
- Social connection and community engagement
- Care coordination and partnership working
- Signposting
Shropshire community-led mental health outreach
Delivered by Shropshire Mental Health Support and Community Resource, including one-to-one outreach, peer support, cafés and buddying for individuals and their family carers. This will include:
- One-to-one mental health outreach
- Calm cafes
- Neurodivergent cafe
- Peer support groups
- Mental health education and training
- Practical and emotional wellbeing support
- Social connection and community engagement
- Care coordination and partnership working
- Buddy system
- Signposting
Live Well Shropshire
Offering social and physical activities, befriending and help to build everyday skills and confidence of individuals and their family carers. This will include:
- Initial assessment, personalised planning and review
- One-to-one wellbeing support
- Group-based daily living skills support
- Social and community activity groups
- Physical activity and falls-prevention support
- Health and wellbeing education
- Digital inclusion support
- Peer buddy and befriending support
- Assistive technology confidence building
- Online wellbeing tools and communities
- Care coordination and partnership working
- Signposting
How to make a referral
Referrals are managed through the central point of referral, coordinated by Shropshire Council, and are triaged by council navigators who match individuals to the most appropriate support.
You can refer if you work in:
- Health services
- Adult social care
- Primary care and social prescribing
- Housing services
- Voluntary and community organisations
With their consent, you can refer a person into the service by calling: 0345 678 9012 and selecting option 2.
The service will also be able to receive referrals electronically soon.
What happens next?
Once a referral is received:
- The referral is reviewed by the central point of referral team
- The individual is matched to the most appropriate support
- Provider/s deliver coordinated, person-centred support
Where appropriate, individuals may move between different types of support without needing a new referral.
The Shropshire Wellbeing and Navigation Network is a preventative service and isn't suitable for adults who:
- Require urgent or emergency support
- Are at immediate risk of harm to themselves or others
- Require specialist or secondary mental health services
- Need crisis intervention or safeguarding responses

- Adult social care assessments
- Clinical mental health services
- Emergency or crisis services
